Preheat oven to 350°F. Coat an 8-inch-square baking dish with cooking spray.
Place apples in the bottom of the prepared dish; drizzle with lemon juice. Top with raisins, cinnamon and nutmeg.
In large bowl, whisk flour, baking powder, baking soda and salt. In a small bowl, blend egg, sugar, 1/4 cup milk, 1/4 cup melted butter and vanilla.
Make a well in the center of the flour mixture and stir in egg mixture. Pour batter over apples in baking dish.
In a small saucepan, combine the remaining 1/4 cup milk, 2 tablespoons cold butter and caramel sauce; bring to a boil, then remove from heat and pour over the batter in baking dish.
Bake for 40 to 45 minutes or until a toothpick tests clean. Spoon out servings and serve with whipped cream, if desired.
